New application procedure and requirements:
- Existing beer permit number will not change.
- A $250 application fee will be applied to the existing permit for payment using the online payment system - ePermits.
- Application will be added to the next available Beer Board meeting agenda. The standard operating procedure for board meetings will be followed.
- Required documents:
- Business License
- Certificate of Registration (Sales and Use Tax)
- Beer Board Initial Inspection
Please complete the initial inspection form for a name change and email it to [email protected]. The form can be found at Beer Permit Requirements. (Please choose the correct form for your permit type.)
- Once all required documents have been submitted and the DBA Name Change has officially occurred, we may issue a temporary beer permit under the new business name.
- The beer permit will be linked to the previous permit, including the history at that location(i.e., inspections, complaints, citations, etc.).
