Death certificates are available for those deaths that occurred in Metropolitan Nashville and Davidson County, Tennessee beginning January 1, 1966 to the present. The death certificate is a copy of the original.
Identification Requirements
Identification is required to obtain a certified copy of a death certificate. An example of proper identification is a copy of your valid driver's license. Please send this in along with your completed application and payment.
Eligibility for receiving a certified death certificate with the cause of death includes parent, child, or spouse. All others will have to provide valid documentation to support why it is needed. All requestors must have a valid Government issued photo ID.
Fees
- Death Certificates: $15
- Each additional copy of death record: $15
- Death record copy issued to another government agency: $15

Get Copies in Person
Applications for death certificates are accepted between 8:00 a.m. and 4:00 p.m. Monday through Friday at the Vital Records office.
Get Copies by Mail
To apply for death certificates please print a copy of the application form and mail it to Vital Records.
