On behalf of the applicant organization, I acknowledge that this application in no way obligates Metro Government to perform services related to this request. The applicant will be responsible for all contracting and costs associated with required engineering services and construction activity. These contracting activities should not occur before this application has been approved by Metro staff. All work performed at the direction of the applicant will be reviewed by Metro staff and will be subject to revision and correction at cost to the applicant.

Per Metro Code, this application is to be accompanied by a preliminary support petition from ten (10) property owners on the street and the council member.

By signing this petition, I indicate support for traffic calming measures to be installed on my street. I understand that the specific design of the traffic calming devices have not yet been developed, but may consist of devices which are designed to be traversed at approximately 10-20 MPH. This installation will be funded by my neighborhood or other private sources and not by Metro. I attest that I am the owner of the property at the address provided.
