One man died and four other persons were injured in Thursday night’s two-vehicle crash on I-40 east near the White Bridge Road exit in West Nashville.
The 7:19 p.m. collision involved a 2013 Honda CRV and a 2016 Dodge Ram pickup truck. The driver of the Honda, Jacabo Hernandez, 29, of Elm Hill Pike, was ejected and died at the scene. His passengers, a 28-year-old woman and an 18-month-old boy, received non-life threatening injuries.
The investigation shows that Hernandez was traveling east on I-40 when he lost control of the vehicle in heavy rain. The Honda struck the center median wall and came to rest in a travel lane of the interstate. Hernandez had transitioned to the back seat to check on the child, who was in a car seat, when the Honda was struck in the rear by a Dodge Ram pickup truck. Hernandez was ejected through a rear window and died at the scene.
The 24-year-old male driver of the pickup truck was not injured. His 25-year-old female front seat passenger, and his 15-year-old male rear seat passenger, received non-life threatening injuries.
Preliminarily, the contributing factor to this fatal crash appears to be the Honda’s failure to maintain its travel lane.
